Students must complete the application form for Pharmacy Office and Hospital, specifying clearly the places in order of preference (please, indicate the number of identifier that appears in the listings), according to the following instructions:

1º Go to the website of the Faculty of Pharmacy.

2º Click in Login.

3º Log in with your ucm e-mail address and your password.

4º Without closing the we browser, click in the following link for the ellection of the Pharmacy Office: http://goo.gl/forms/VPgCk9C7MdD9GBK62

Fill up to 50 pharmacy offices using the proper identifier and send the form asking a copy of the application form.

5º Click in the following link to apply for a place in a hospital: http://goo.gl/forms/B9rViW8dcBRvpwIg1

fill up to 30 hospitals using the proper identifier and send the form asking a copy of the application form.

All the students will carry out 3 months of stage in a hospital and 3 moths in a pharmacy office. This is the reason why it is very important to fill both forms

GENERAL RULES AND GUIDELINES:

GENERAL REQUIREMENTS:
To have passed all the subjects of the first three undergraduate courses.
To have enrolled all the subjects to finish the studies (except the Final Degree Project).
Not having more than 75 ECTS or credits enrolled.
Not having any subjects enrolled in the semester that the practices are carried out (for the students of Degree).
To have at most 2 subjects enrolled in the semester that the supervised practices are carried out (for old Degree -licenciatura- students). 

PERIOD 2: (students enrolled in Group B): from February 13 to July 14.
